How to Create a Form to Subscribe Contacts to SMS Marketing Lists

In this guide, you'll learn how to create a form in the CRM that collects contact details and adds subscribers directly to your SMS marketing list.

Important Notes Before You Start

  • Create your SMS marketing list first. You won’t be able to connect a form to an SMS list unless it already exists.

  • Form style options depend on your CRM plan. Some styles and custom field features may not be available on all plans.

  • Your form type affects your design and settings. For example, inline forms offer more customization than floating bar forms.

  • Preference Center forms only work with email lists. They cannot be used for SMS subscriptions.

Step-by-Step: Create a Form for SMS Subscriptions

  1. Go to the Forms section
    From the left-hand menu in your ActiveCampaign account, click Website > Forms.

  2. Start a new form

  3. Name your form
    In the “Form Name” field, type in a name that helps you identify it later (e.g., SMS Promo Sign-Up). 

  4. Choose your form style
    Pick a form style from the options provided (e.g., inline, floating bar). Image 4-8-25 at 3.30 AM

    Remember: The available styles may vary depending on your plan.

  5. Set up the form action
    Click the "Form Action" dropdown and choose “Subscribe to SMS list.”

    💡 You can add multiple actions to your form. For example, in addition to subscribing to an SMS list, you can also add a tag or send a notification email.

  6. Select the SMS list
    Under “Subscribe to SMS list,” choose the specific SMS list you want new contacts to be added to.

    ⚠️ The "SMS Consent Message" checkbox will be greyed out and already selected. This is required by law to confirm that contacts agree to receive SMS messages.

  7. Create your form
    Click the "Create Form" button. This opens the form builder.

  8. Customize your form
    Use the drag-and-drop builder to add fields like:

    • First name

    • Phone number

    • Custom fields (if supported by your plan)

Imagine this:
You're running a promotion and want people to sign up to get text alerts. You create a form called “VIP SMS Masterlist”, choose an inline style, and set the form action to subscribe people to your “VIP SMS Masterlist.”

Now, when people fill out the form and enter their phone number, they’re automatically opted in to receive your SMS campaigns — with proper consent already built in!
